Diversion Resource Navigator in Berks County


United Way of Lancaster is looking for a Diversion Resource Navigator in Berks County to join our PA 211 East team. About 211

Pennsylvania 211 East is a free, confidential, non-emergency, comprehensive information and referral service that connects Berks and Lancaster County residents with the health and human services they need.

About The Position

This position is responsible for providing information, referral and prescreening for Berks County Housing programs. Exceptional customer service and accurate data recording is essential in this position, while acting in accordance with our values, vision, and mission. This position is full-time and will be in person and traveling to sites. Must be flexible with the scheduled hours.

Responsibilities

  • Help to implement diversion programs, ensuring compliance with policies, procedures, and legal requirements
  • Track program capacity, participant progress, and service utilization
  • Point of contact for referrals
  • Conduct intake assessments to identify participant needs, risks, and strengths
  • Develop individual diversion plans and connect participants to appropriate services
  • Maintain regular contact with participants to monitor progress, address barriers,
    and encourage engagement
  • Provide training and presentations to staff and partners about diversion program
  • goals, eligibility, and processes
  • Maintain accurate, confidential case records and documentation
  • Track program outcomes and prepare reporting for internal leadership
  • Use data to identify trends, improve program effectiveness, and support grant
    reporting
  • Use de-escalation techniques and trauma-informed practices

Qualifications

  • High School Diploma or GED required
  • Degree in Social Work or equivalent experience preferred
  • Bilingual in Spanish and English required
  • Valid Driver’s License is required
  • Reliable transportation required
  • Travel between Berks County locations required
  • Clearances are required with relevance to the position

Employment

This is a full-time hourly, non-exempt position. The pay rate for this position is $22/hour.
